With an eye towards moving residents and caregivers to a more home-like environment, the Neighborhoods at Brookview facility moves from a medical model of care to a social model. With the social model of care, residents have active choices in their daily lives and activities, providing a sense of independence.
The Neighborhoods at Brookview consist of three neighborhoods of two households each (79 skilled beds), with a town center, support spaces, and therapy area. The six households have 13 to 14 residents each, with five households providing traditional long-term care and one household providing Medicare transitional care. The new care center lives on the same campus as the hospital.
